Quick answer: There is no Medicare grocery allowance that every person on Medicare can claim. Original Medicare (Parts A and B) has no food benefit. Some private Medicare Advantage plans load money for healthy food onto a prepaid card, but in 2026 only 11% of individual plans offer a food and produce benefit, compared with 85% of Special Needs Plans.1 If you need food help now, start with our guide to food help for seniors.

This guide covers who qualifies, how a food card differs from an OTC card or flex card, what changed in 2026, and how to spot the TV-ad and phone scams.

Is the Medicare grocery allowance real?

Yes, but only as an extra benefit inside certain Medicare Advantage plans. It is not a government card, and Medicare does not mail it to everyone.

Medicare Advantage plans are private plans that can offer extras that Original Medicare does not cover.1 Since 2020, federal law has let these plans offer "Special Supplemental Benefits for the Chronically Ill" (SSBCI). These are extras that are not strictly medical, such as food and produce.2 A "grocery allowance," "healthy food card" or "food card" is usually this benefit.

  • Who pays for it: your Medicare Advantage plan, not Medicare directly.
  • Who sets the amount: each plan sets its own maximum dollar amount.2
  • How it arrives: often as a dollar amount on a flex card or spending card. It may expire at the end of each month or roll over until the end of the year.1

Can everyone on Medicare get a grocery allowance?

No. You need to be in a plan that offers the benefit, and you need to meet that plan's health rules.

To get SSBCI, you must meet all three parts of the federal "chronically ill enrollee" test.2

  1. You have one or more serious chronic conditions that are life-threatening or that greatly limit your health or daily function.
  2. You have a high risk of hospital stays or other poor health outcomes.
  3. You need intensive care coordination.

The food item must also have a reasonable chance of keeping you healthier. A plan may look at things like income or living situation, but it cannot use them as the only reason you qualify.2 Having a chronic condition does not automatically mean you qualify.7

Who is eligible for the Medicare allowance card in a Special Needs Plan?

Special Needs Plans (SNPs) are Medicare Advantage plans for specific groups. You must have Part A and Part B and live in the plan's service area.5 SNPs are not offered everywhere.5

Plan typeWho can joinFood benefit in 2026
Original Medicare (Parts A and B)Everyone on MedicareNone1
Standard Medicare Advantage planAnyone with Parts A and B in the service area11% of plans1
D-SNP (Dual Eligible SNP)People with both Medicare and Medicaid5Common: 85% of all SNPs offer food and produce1
C-SNP (Chronic Condition SNP)People with a qualifying chronic condition5Same 85% SNP figure1

Source: KFF analysis of CMS plan files for 2026; Medicare.gov. KFF reports SNPs as one group.

CMS lists 15 conditions that can qualify someone for a C-SNP. They include diabetes, chronic heart failure, dementia, end-stage renal disease on dialysis, HIV/AIDS, stroke, cancer, certain lung disorders such as emphysema and chronic bronchitis, and certain mental health conditions.6 Even inside an SNP, each person still has to meet the plan's SSBCI rules to get the food benefit.2

Is there a Medicare grocery card for 2026?

Only through qualifying plans, and fewer people have one than in 2025. The biggest change is that a CMS test program called the Value-Based Insurance Design (VBID) Model ended on December 31, 2025.3

Under VBID, plans could target extras such as grocery help by low-income status (the Low-Income Subsidy), by chronic condition, or by where a person lived.3 That let plans give food benefits based on income alone, which SSBCI does not allow.1 CMS ended the model because it found $2.3 billion in extra Medicare costs in 2021 and $2.2 billion in 2022.4

The share of individual plans with a food and produce benefit fell from 15.0% in 2025 to 11.1% in 2026.1 If your card stopped working for food in January 2026, this change is a likely reason.

OTC card vs. flex card vs. healthy food card: what's the difference?

These names overlap, and plans use them differently. The key question is which benefit is loaded on your card, not what the card is called.

Card nameWhat it usually pays forWho gets it
OTC cardAn allowance for over-the-counter items66% of individual plans and 94% of SNPs offer it in 20261
Healthy food card / grocery allowanceFood and produce, such as fresh produce, frozen foods and canned goods2Members who meet SSBCI rules2
Flex cardOne dollar amount for several benefits, which may include food, utility bills, rent and non-medical rides1Depends on which benefits the plan loads

The OTC allowance is the most common of the three, but it is for over-the-counter items, not groceries.1 Many people see "OTC card" in an ad and assume it buys food. It only does if the plan also loads a food benefit on the same card.

What can I buy with an OTC card or food card?

Each plan publishes its own list of approved items and stores. Federal rules set the outer limits:

  • Food benefit: may include produce, frozen foods and canned goods.2 Tobacco and alcohol are not allowed.2 Plans cannot offer non-healthy food as SSBCI.7
  • OTC benefit: covers over-the-counter items on the plan's list.1

Can I buy toilet paper with my OTC card?

Only if your plan's item list includes it. Paper goods are not food, so a food benefit will not cover them.2 Check your plan's OTC catalog or call member services before you shop.

How much money is on a flex card?

It depends on the plan. There is no single federal amount; each plan sets its own maximum.2 Ads that name one figure for "everyone on Medicare" do not match how the benefit works.

How do I find out if my plan has a grocery allowance?

Your plan's own documents are the only reliable answer. Plans must state in their Evidence of Coverage what the extra benefit covers and how they decide who qualifies.2

  1. Read your Evidence of Coverage. Search it for "SSBCI," "food," "produce" or "healthy food."
  2. Call the member services number on your plan ID card. Ask: "Does my plan offer a food and produce benefit, and do I qualify?"
  3. Ask what proof they need. Plans are expected to use objective criteria, such as health risk assessments or claims data, to decide eligibility.2
  4. Compare plans on Medicare Plan Finder if you are thinking of switching.

When can I switch to a plan with a food benefit?

Enrollment periodDatesWhat you can do
Open EnrollmentOctober 15 to December 7; changes start January 1Join, drop or switch Medicare Advantage plans8
Medicare Advantage Open EnrollmentJanuary 1 to March 31Make one switch if you are already in a Medicare Advantage plan8
C-SNP Special EnrollmentAny timeJoin a C-SNP for your condition, once8
Integrated D-SNP Special EnrollmentOnce a monthFor people with Medicare and full Medicaid8

A food card is one small part of a plan. Switching also changes your doctors, drug coverage and costs, so compare the whole plan before you move.10

How to spot a Medicare grocery allowance scam

Medicare does not issue flex cards.10 Any ad or caller that says "Medicare is giving out grocery cards" is wrong, and some are scams.

Watch for these warning signs:

  • A call "from Medicare." Medicare will never call you to sell you anything or come to your home.9
  • A request for your Medicare or Social Security number to "send your card." Thieves use these numbers for identity theft or false Medicare claims.10
  • A cold call from a plan salesperson. Plan reps may not call you unless you are a member or gave permission.10
  • "Use it for anything." Real food benefits have strict item and eligibility rules.10 Plans are also not allowed to market SSBCI as guaranteed.2
  • Pressure to switch plans over the phone. Accepting a card can quietly move you out of Original Medicare.10 Medicare advises never joining a plan over the phone unless you made the call.9

If you think you were targeted, call 1-800-MEDICARE (1-800-633-4227) or report it online to the HHS Office of Inspector General. If you are in a Medicare Advantage or drug plan, you can also call 1-877-772-3379.9

What if my plan has no food benefit?

Other programs can help with groceries, and most do not depend on your Medicare plan:

How we checked this

On October 12, 2026, we read the CMS guidance on SSBCI and the CMS pages on the VBID Model and C-SNP conditions. We also read Medicare.gov pages on Special Needs Plans, enrollment periods and fraud, plus the HHS Food Is Medicine hub, KFF's 2026 plan analysis and AARP's scam guidance. We queried the PantryPath directory the same day (confirmed listings with a state, n=7,566).11 We did not test any plan's card or review any company's benefit amounts, which vary by plan and county.

Next step: if your plan does not cover food, see every food program for older adults or search for a pantry near you.
